Supports Planning
The Affordable Care Act changed certain community-based services for individuals. Changes in Medicaid eligibility guidelines effective 01/01/14 have qualified more individuals for State Medicaid. The programs offer both self-direction and agency model services.
Supports planners engage participants in a person centered planning process to identify appropriate services to meet determined needs. Coordinate community services and supports. Assists the applicant in developing a comprehensive plan of service. Conduct quarterly face to face visits and monthly contact with each participant.
Community Personal Assistance Services (CPAS): Participants eligibility requirements are that participants must need assistance with one (1) activity of daily living, no age limitations and meet Medicaid eligibility guidelines. The following services may be covered under the CPAS program:

Community First Choice (CFC): Participants must meet an institutional level of care, no age limitations and meet Medicaid eligibility guidelines. The following services may be covered under the CFC program:

- Home-Delivered Meals
- Assistive Technology
- Environmental Assessments
- Accessibility Adaptations
- Consumer Training
- Personal Emergency Response Systems
- Transition Services

The Waiver for Older Adults has been merged with the Living at Home Waiver and they are renamed the Home and Community Based Options Waiver (HCBOW): Participants must meet a nursing home level of care, aged 18 and over and meet waiver financial eligibility guidelines. The following services may be covered under the HCBOW program:

- Nurse Monitoring
- Home-Delivered Meals
- Assistive Technology
- Environmental Assessments
- Accessibility Adaptations
- Consumer Training
- Personal Emergency Response Systems
- Transition Services
- Medical Day Care

- Family Training
- Behavioral Consultation
- Senior Center Plus
- Assisted Living
Currently there is a waiting list for community applicants for HCBOW. However, interested community applicants can contact the MAP program at 301-783-1752. Nursing home applicants that have a current nursing home level of care and long term care Medicaid should contact the nursing home social services department to begin the process.
